What does Initiating event mean?
In safety and regulatory practice, an initiating event is the first abnormal occurrence that triggers a potential accident sequence in a plant or system. It is a descriptive engineering expression (often called an “initiating cause”), not a term defined in legislation or case law, but it frequently appears in safety cases, risk assessments and expert evidence under the COMAH/Seveso regimes, nuclear site licensing, offshore safety cases and general health and safety enforcement. An initiating event is a credible challenge to normal operation—such as equipment failure, human error, loss of utilities or an external hazard—from which consequences and outcomes are evaluated. event tree Analysis, and related methods, start with identified initiating events to map possible progressions and the effectiveness of protective barriers. Legally, its significance lies in foreseeability, adequacy of hazard identification, and whether risks are reduced so far as is reasonably practicable (ALARP). Dutyholders are expected to identify initiating events, estimate frequencies, and demonstrate controls in safety documentation. Usage is broadly consistent across England & Wales, Scotland and Northern Ireland (COMAH 2015/NI) and Ireland (Seveso III Regulations 2015), with terminology reflected in regulator guidance (HSE, ONR, HSENI, HSA) rather than in primary legislation.
